ルミパルスPrestoIIを用いた改良型PIVKA-II測定の基礎的・臨床的検討

内容

「ルミパルスプレストPIVKAⅡエーザイ」の基本性能を評価し、肝切除術に用いる止血シートに由来する偽高値が回避できているかを検証した。再現性、希釈直線性、相関の基本的性能は良好であった。また、フォルテで偽高値を示した11例をプレストで測定した結果、1例を除き、ほぼ偽高値は回避されていた。偽高値が回避できていない症例に対しても、活性の高い不活化ALPを用いた改良型試薬では改善がなされていた。

Whispering Gallery Mode Biosensor Operated in the Stimulated Emission Regime

Whispering gallery modes (WGMs) are generated in fluorescent polymer latex beads immersed into phosphate buffered saline (PBS) above the threshold for stimulated emission by means of an ultrashort pulse laser and compared to their characteristics below threshold. The WGM microresonators show an eight-fold improvement of their Signal-to-Noise ratio and a three-fold increase in their quality factor when operated above threshold. In a first investigation on the benefits of these improvements for biochemical sensing, a real-time kinetics of Bovine Serum Albumin adsorption onto the resonator surface in PBS is monitored and compared with those kinetics acquired by means of a WGM microresonator operated below threshold as well as by surface plasmon resonance.

Correlation between Serum Hepatitis B Virus Core-Related Antigen and Intrahepatic Covalently Closed Circular DNA in Chronic Hepatitis B Patients

Nucleos(t)ide analogues are utilized for the treatment of chronic HBV infection, and HBe seroconversion, and HBV DNA levels are commonly used as markers of viral status and of primary end points of treatment. Recently, a new assay was prepared for the detection of serum HBV core-related antigen (HBcrAg), consisting of HBcAg, HBeAg and p22cr, by coding the precore/core region. In this study, we examined the correlation between serum HBcrAg concentration and viral status by the analysis of serum HBeAg, HBsAg, peripheral HBV DNA and intrahepatic cccDNA in 32 chronic hepatitis B patients. Intrahepatic cccDNA was detected in all 32 cases, 22 cases showed HBcrAg-positive, and serum HBcrAg concentration level was closely correlated with cccDNA. Additionally, positive HBcrAg concentration level results were observed in 6 out of 13 HBsAg seroclearance cases and 11 out of 20 HBV DNA-negative cases. Moreover, the correlation between HBcrAg and cccDNA in these 20 HBV DNA-negative cases was statistically significant (r=0.632, p<0.003). These data suggest that serum HBcrAg concentration is well correlated with intrahepatic cccDNA level, and that the measurement of serum HBcrAg may be clinically useful for monitoring intrahepatic HBV viral status, especially in patients under treatment with nucleos(t)ide analogues.

Fabrication of Versatile 3-D Ternary Nanostructures via Gas-mediated Metal Evaporation

By combining Nanosphere Lithography (NSL) with metal evaporation at two different chamber pressures, ternary metallic nanostructures with 3D topography were fabricated and characterized by Scanning Electron Microscopy (FEG-SEM) and atomic force microscopy (AFM). The nanstructures consist of 340 nm silicon patches surrounded by a chromium mesh of varying height and gold triangular pillars with a height of 40 nm and diameter of about 200 nm. While NSL was due employed due to its ease of use, evaporation at different chamber pressures can be combined with any kind of template mask comprising convex shape to yield novel kinds of ternary nanostructures analogous to the ones presented here. The method shows to be more versatile than plasma based deposition techniques due to avoidance of substrate charging effects and a freely adjustable chamber pressure.

B型肝炎ウイルスの発見は、肝臓病の診断検査、治療、研究に大きく寄与したが、発見から40年以上が経過した現在でもHBVとそれに起因する肝疾患に関してはまだ不明な点も多い。
本稿では、B型肝炎ウイルスの構造、感染、ウイスル増殖、検査、治療、予防等の基礎についてと、新たに保険適用になったHBコア関連抗原検査の臨床的有用性について具体的データを示して紹介する。

Metabolic syndrome is regarded as visceral fat accumulation associated with a cluster of cardiovascular risk factors such as hyperglyceridemia, insulin resistance, and hypertension. A combination of markers including waist circumference, triglyceride, high-density lipoprotein cholesterol and blood pressure are used for the clinical diagnosis of metabolic syndrome. A number of recent reports have indicated that apolipoprotein B-48 (apo B-48), a component of chylomicron and chylomicron remnants, is a parameter for evaluating lipoprotein metabolism in the exogenous pathway. Additionally, Adiponectin is an adipocyte-specific protein secreted from adipose tissues as a trimer, hexamer or high-molecular weight (HMW) complex, and the concentration of the HMW form is correlated with insulin resistance and metabolic disorders. Therefore, it seems likely that apo B-48 and HMW adiponectin constitute useful metabolic syndrome markers.

近年、脂肪細胞から産生されるアディポサイトカインが注目されてきており、中でもアディポネクチンが内臓脂肪蓄積を中心病態とし、インスリン抵抗性、動脈硬化などとの関連から注目されてきている。また、血液中には低分子量体(LMW)、中分子量体(MMW)および高分子量多量体(HMW)アディポネクチンが存在するが、この中でも高分子量多量体(HMW)アディポネクチンがその生理活性を担っていると報告されている。本稿では、血管系を中心とした病態におけるアディポネクチン、特に高分子量多量体(HMW)アディポネクチンの役割および高分子量多量体(HMW)アディポネクチンを特異的に測定可能な測定試薬について紹介する。

Allostery: absence of a change in shape does not imply that allostery is not at play

Allostery is essential for controlled catalysis, signal transmission, receptor trafficking, turning genes on and off, and apoptosis. It governs the organism's response to environmental and metabolic cues, dictating transient partner interactions in the cellular network. Textbooks taught us that allostery is a change of shape at one site on the protein surface brought about by ligand binding to another. For several years it has been broadly accepted that the change of shape is not induced; rather, it is observed simply because a larger protein population presents it. Current data indicate that while side chains can reorient and rewire,allostery may not even involve a change of (backbone) shape. Assuming that the enthalpy change does not reverse the free energy change due to the change in entropy, entropy is mainly responsible for binding.
